In your crockpot place the chicken, chicken broth, cream cheese, heavy cream and butter.
Allow to cook on high 3-4 hours or low 6. Stir occasionally, if you can.
Remove chicken from crockpot. Add veggies if desired. If using veggies that are chunky, make sure you break them up. Cook pasta a little less than al dente.
Shred or dice up cooked chicken.
Add cooked pasta. Stir.
Add shredded mozzarella and stir, if desired. Sprinkle the top with shredded Parmesan. Allow to melt and then serve. You can top with red pepper flakes, Italian seasoning, or parsley, if desired.
